Klauzula zgody
„Wyrażam zgodę na przetwarzanie moich danych osobowych w zakresie imienia, nazwiska oraz adresu e-mail przez Uniwersytet Gdański z siedzibą w Gdańsku (80-309) przy ul. Bażyńskiego 8, w celu rejestracji na wydarzenie Inellectual Property and Innovation Week. Ponadto oświadczam, iż zostałem poinformowany o możliwości wycofania zgody w dowolnym momencie oraz że jej wycofanie nie wpływa na zgodność z prawem przetwarzania, którego dokonano na podstawie zgody przed jej wycofaniem.”
Przesyłając formularz rejestracyjny deklaruję chęć uczestnictwa w wydarzeniu oraz oświadczam, że zapoznałem (-am) się z informacją o przetwarzaniu danych osobowych, a także jestem świadomy (-a), że biorąc w nim udział wyrażam zgodę na przetwarzanie przez Organizatorów mojego wizerunku utrwalonego podczas wydarzenia oraz jego publikację na stronach Internetowych Organizatorów, newsletterach Organizatorów i w mediach społecznościowych, na portalach Facebook, LinkedIn, YouTube itp. w celach informacyjnych, promocyjnych i reklamowych, a także na potrzeby dokumentowania działań np. w prezentacjach. Wiem, że udzieloną zgodę mogę w dowolnym momencie wycofać.
RODO
Zgodnie z ogólnym rozporządzeniem o ochronie danych z dnia 27 kwietnia 2016 r. zwanym dalej RODO informujemy, iż:
1. Administratorem Pani/Pana danych osobowych jest Uniwersytet Gdański z siedzibą w (80-309) Gdańsku przy ul. Jana Bażyńskiego 8.
2. Administrator powołał Inspektora Ochrony Danych, z którym można skontaktować się pod numerem telefonu (58) 523 31 30 lub adresem e-mail: iod@ug.edu.pl. Z Inspektorem Ochrony Danych można kontaktować się we wszystkich sprawach dotyczących przetwarzania danych osobowych oraz korzystania z praw związanych z ich przetwarzaniem.
3. Uniwersytet Gdański będzie przetwarzał następujące dane osobowe – 3. Uniwersytet Gdański będzie przetwarzał następujące dane osobowe – imię i nazwisko, miejsce pracy oraz adres e-mail.
4. Pani/Pana dane osobowe przetwarzane będą w celu rejestracji na wydarzenie Inellectual Property and Innovation Week.
5. Podstawą prawną do przetwarzania Pani/Pana danych osobowych jest art. 6 ust. 1 lit. a RODO – przetwarzanie na podstawie zgody.
6. Podanie przez Panią/ Pana danych osobowych jest niezbędne do rejestracji na wydarzenie Inellectual Property and Innovation Week.
7. Pani/Pana dane osobowe będą przetwarzane w imieniu administratora danych przez upoważnionych pracowników wyłącznie w celach, o których mowa w ust. 4.
8. Pani/ Pana dane osobowe będą przechowywane przez cały okres obejmujący czas przyjmowania formularzy zgłoszeniowych, przygotowania, realizacji oraz rozliczenia warsztatów naukowych, a także później w związku z realizacją obowiązku archiwizacyjnego – nie dłużej jednak niż do czasu wycofania zgody..
9. Pani/Pana dane osobowe nie będą udostępniane podmiotom zewnętrznym z wyjątkiem przypadków przewidzianych przepisami prawa.
10. Na zasadach określonych przepisami RODO przysługuje Pani/Panu:
10.1. prawo do cofnięcia zgody w dowolnym momencie bez wpływu na zgodność z prawem przetwarzania, którego dokonano na podstawie zgody przed jej cofnięciem,
10.2. prawo dostępu do treści swoich danych,
10.3. prawo do ich sprostowania, gdy są niezgodne ze stanem rzeczywistym,
10.4. prawo do ich usunięcia, ograniczenia przetwarzania, a także przenoszenia danych – w przypadkach przewidzianych prawem,
10.5. prawo do wniesienia skargi do organu nadzorczego – Prezesa Urzędu Ochrony Danych Osobowych, gdy uzna Pani/Pan, że przetwarzanie Pani/Pana danych osobowych narusza przepisy o ochronie danych osobowych.

“I consent to the processing of my personal data in the scope of name, surname and e-mail address by the University of Gdansk, based in Gdansk (80-309) at 8 Bażyñskiego Street, for the purpose of registration for the event „Inellectual Property and Innovation Week”. In addition, I declare that I have been informed that I can withdraw my consent at any time and that the withdrawal of consent does not affect the legality of the processing performed on the basis of consent before its withdrawal.”
Sending the registration form, I declare my willingness to participate in the event, and I declare that I have read the information on personal data processing, and I am aware that by taking part I agree to the Organizers’ processing of my image captured during the event and its publication on the Organizers’ websites, the Organizers’ newsletters and social media, on Facebook, LinkedIn, YouTube, etc. for informational, promotional and advertising purposes, as well as for the purpose of documenting the activities e.g. in presentations. I know that I can withdraw the consent I have given at any time.
GDPR
.In accordance with the General Data Protection Regulation of April 27, 2016, hereinafter referred to as RODO, we inform you that:
1.The Administrator of your personal data is the University of Gdansk, based in (80-309) Gdansk, 8 Jana Bażyńskiego Street.
2. the Administrator has appointed a Data Protection Officer, who can be contacted at (58) 523 31 30 or e-mail address: iod@ug.edu.pl. The Data Protection Supervisor can be contacted in all matters concerning the processing of personal data and the exercise of rights related to data processing.
3 The University of Gdansk will process the following personal data – name, place of work and e-mail address.
4. your personal data will be processed for the event „Inellectual Property and Innovation Week”.
5.The legal basis for processing your personal data is Article 6(1)(a) RODO – processing on the basis of consent.
6. The provision of personal data are necessary for registration for the event „Inellectual Property and Innovation Week”.
7. Your personal data will be processed on behalf of the data controller by authorized employees only for the purposes referred to in paragraph 4.
8. Your personal data will be retained for the entire duration of the acceptance of the application forms, the preparation, execution and settlement of the scientific workshops, as well as thereafter for the fulfilment of archiving obligations – but no longer than until you withdraw your consent..
9. Your personal data will not be shared with external entities except as provided by law.
10. under the terms of the provisions of RODO, you have:
10.1. the right to withdraw your consent at any time without affecting the legality of the processing carried out on the basis of consent before its withdrawal,
10.2. the right to access the content of your data,
10.3. the right to rectify them when they are inconsistent with the actual state,
10.4. the right to erasure, limitation of processing, as well as data portability – in cases provided by law,
10.5. the right to lodge a complaint to the supervisory authority – the President of the Office for Personal Data Protection, if you consider that the processing of your personal data violates the provisions on personal data protection.
